STATE OF TEXAS APPLICATIONS -- UPDATED 9/1/2009

Below are the links to the accessible versions of the revised state application, instructions for the application and the continuation sheet which became effective September 1, 2009.

As mentioned in the August 24, 2009 letter to State Agency Heads, these forms are now available for download on the Texas Workforce Commission website. For compatibility purposes, the documents are in Word 97-2003. Individuals utilizing a later version of Word, such as 2007, should be able to access and use the Word 97-2003 version successfully.

It is recommended that users save the application to their desktop before filling out the application. This should ensure that there are no formatting issues that sometimes occur when the application is accessed using a web browser.
